If someone other than the landowner submits an application a legal representation document must be signed by the owner and legal representative. This document includes information for the parcel of land on which the septic will be installed, as well as contact information for the owner and their legal representative.
Site plans are required for all applications and will need to be submitted with the property dimensions, a proposed home with the dimensions, setbacks to property lines, a driveway location, proposed well and septic area. If the property is not vacant you will need to include all existing structures, wells, and septics on the site plan.
FLOOD PLAIN DETERMINATION:
A flood plain determination will need to be done by the Planning Department for all new permits, or any permits not previously submitted to Environmental Health on a parcel of land. The top portion of the form will need to be filled out and the Planning Department will complete the bottom. The form will need to be submitted to Planning in person.
