A septic system repair permit requires:
A completed application .
A site plan.
A completed homeowner interview form.
A flood plain determination (see below).
NO FEE: There is no charge for repair permits.
If someone other than the land owner submits an application a legal representation document must be signed by the owner and legal representative. This document includes information for the parcel of land on which the repair will be made, as well as contact information for the owner and their legal representative.
FLOOD PLAIN DETERMINATION: A flood plain determination will need to be done by the Planning Department for all new Permits, or any permits not previously submitted to Environmental Health on a parcel of land. The top portion of the form will need to be filled out and the Planning Department will complete the bottom. The form will need to be submitted to Planning in person.
