REMOVE DECEASED VOTER:  To remove a voter from the voter registration list that has been deceased. The Board of Elections office can only remove deceased voters with a written authorization from a near relative OR information from the Department of Health and Human Services OR a death certificate.  Click here for a pdf version of the form.  Click here for a Word version of the form.

CANCELLATION OF VOTER REGISTRATION: This form is intended to provide notification of a voter's request to cancel his or her voter registration.  This form can only be completed by the voter and must be signed.  Click here for form.
