Provision of personal health care services to assist clients in maintaining healthy lifestyles through preventive health screenings and exams, early detection and treatment of chronic disease, and referral to appropriate human service providers.
FEES & INSURANCE:
Fees charged for most services are based on a sliding scale according to income guidelines and/or availability of insurance coverage (private, Medicare and Medicaid). Most insurances accepted. Click here for a current list of insurance providers accepted at our clinics. All clients will need to bring in proof of income so that we may apply our sliding scale and determine your total charges for the visit.
For clients with insurance, you will be expected to pay your co-pay at the time of your visit. If you are unable to pay your co-pay, we will help you reschedule your appointment. If you have any questions, please feel free to call 919-742-5641 to speak with a staff member.

CLINICAL SERVICES PROVIDED:
General Clinical Services
Tuberculosis skin testing, initiation of Tuberculosis medication regimen and refills, pregnancy tests, contraception counseling and method refills, including emergency contraception. Do not have to be a resident of Chatham County; fees vary.
Provide immunizations to children (infants through 18 years) for the prevention of life threatening communicable diseases. Offer tetanus and Tdap vaccines for adults (ages 19+). Fees vary for some immunizations. Please call our office for a current list of adult vaccines available. Do not have to be a resident of Chatham County.
Childhood Lead Poisoning Prevention Program
The Chatham County Public Health Department is dedicated to increasing lead testing of children under six years of age and eliminating lead risks in the county. Click here to go to the childhood lead poisoning prevention page.
Communicable Disease/Sexually Transmitted Infections
Confidential counseling, testing and treatment for communicable and sexually transmitted infections (including HIV testing and counseling). Medicaid can be billed. Do not have to be a resident of Chatham County.
Low-risk prenatal care, coordination of high-risk obstetrical care with UNC, WIC and social work referrals. Fee based on a sliding fee scale; Medicaid, Medicare and insurance can be billed. Must be a Chatham County resident.
Postpartum Newborn Home Visiting Program
Nurse visits new mothers with high-risk babies to complete a health assessment. Medicaid, Medicare and insurance can be billed. There is no charge to client. Referrals accepted from outside providers and hospitals. Must be a Chatham County resident.
Child Health Services
Children from birth to 20 years of age, well-child and sick child health care. Sports and kindergarten physicals are also available for established clients. Fee based on a sliding fee scale; Medicaid and insurance can be billed. Must be a Chatham County resident.
Family Planning Services
Health care screening and maintenance for women of child bearing age, counseling for and provision of birth control methods. Fee based on a sliding fee scale; Medicaid and insurance can be billed. Do not have to be a resident of Chatham County.
Breast and Cervical Cancer Control Program
Physical examinations for women 50-64 years of age who have no or limited health insurance and do not have Medicaid or Medicare. Exams include clinical breast exam, mammogram, and Pap smear, with management and referral as appropriate. Fee is based on income. Must be a Chatham County resident.
Nutrition counseling services available for weight management, high blood pressure, high cholesterol, diabetes, pregnancy, iron-deficiency anemia, and issues with infant feeding. Diabetes Self Management Program also available. Fee is based on a sliding fee scale; Medicaid and insurance can be billed. Must be a Chatham County resident. Do not have to be Chatham County Public Health Department patient. Referrals welcomed from outside healthcare providers.
